Caloric-vHIT dissociation is often linked to Ménière's disease, but its role in vestibular migraine is unclear. This study aimed to analyze caloric-vHIT dissociation in seven patients with definite vestibular migraine. The research was observational and retrospective, involving a cohort of 100 patients. Seven patients with vestibular migraine showed caloric-vHIT dissociation and were followed for about seven years. Among the cohort, A-pattern dissociation occurred in 18% of vestibular migraine patients and in 57% of Ménière's disease patients. B-pattern dissociation was rare, with only 3% in Ménière's disease and 3% in vestibular migraine. The seven cases exhibited clinical variability, implying multiple underlying mechanisms. These included peripheral hypofunction, familial issues, trauma, and central modulation. Notably, vestibular migraine lacked fluctuating auditory symptoms and progressive hearing loss. None evolved into Ménière's disease, suggesting distinct clinical features for vestibular migraine.
