Summary

In migraine research, structural MRI findings vary widely among different groups. This inconsistency may hide important patient-level differences in neuroanatomy. To explore this, we studied 249 adults with migraines and 57 healthy controls using MRI scans. We aimed to summarize individual MRI deviations into understandable factors. Our primary model identified two significant dimensions related to chronicity and clinical burdens. The first factor linked to chronic migraine showed a 1.50 odds ratio. This factor also correlated with higher monthly headache days and MIDAS scores. The second factor reflected cortical-thickness variations and showed a borderline inverse association with chronic migraine. Additionally, it was linked to high deviation burden. The research indicates that normative MRI deviations can be categorized into meaningful factors. These findings may help in understanding chronic migraine better, although further external validation is needed for clinical use.
